Terms of Service

Once an appointment is a schedule, you understand and acknowledge the terms of service indicated below. These are basic terms of service to ensure proper hygiene and a healthy experience with all current and future pets. A receipt will be created indicating you agree to the terms at the location of service. Thank you for your business and hope to see your and your pal(s) soon!

GROOMING RELEASE FORM

Thank  you  for  choosing  Hot Shot Dog Grooming  for  your  pet’s  grooming  needs. Hot Shot Dog Grooming prides  itself  on  providing  the  best  care  for  your  best  friend. Before  grooming,  please read  and  agree  to  our terms of service:

 

– If your pet has any medical problems (seizures, arthritis, collapsing trachea, etc.), please make us aware of them so that we can take the necessary steps to ensure your pet’s comfort. Sometimes grooming can expose a pre-existing condition. We cannot diagnose such a condition, but may advise you to seek veterinary attention.

– If your pet has any history of biting, we require that you notify us before grooming. This will help prevent injury to us and your pet. If your pet has behavioral issues, we are happy to work through them with you.

– If your pet has fleas, we will automatically administer a flea bath. We will notify you if possible. The charge for this service is $15 to $25 depending on the size of your pet. This cost covers the shampoo, time, and cleaning necessary to ensure that no fleas are active in the salon. The treatment will kill the fleas on your pet, but after leaving our salon, your pet will not be protected from any fleas that may be present in your home or pet area.

– If your pet’s coat is extremely matted, it may need to be shaved down. WE WILL ONLY SHAVE YOUR PET WITH YOUR CONSENT. A matted coat prevents air from reaching your pet’s skin and may cause your pet to bite or scratch itself, resulting in irritated skin that does not heal properly. Shaving uncovers these conditions, as well as any other skin problems that may have developed from lack of grooming. Please note that it is difficult to shave  down  a  matted  pet  without  slightly  irritating  the  skin.  There  will  be  an  extra  charge  for  this  service because of the time and additional blades needed.

–  If  you  must  cancel  your  appointment,  please  give  us  as  much  notice  as  possible.  We  understand  that schedules sometimes change, but chronic cancellations may result in a cancellation fee up to the price of the grooming and the inability to book future appointments.

– If we arrive for your appointment and you are not there, you may be subject to a trip fee. Habitual no shows may be subject to a fee up to the price of the grooming and may not be able to schedule future appointments.

– If your check is returned, you will be charged a $25 fee.

– Hot Shot Dog Grooming reserves the right to refuse service to customers whose pets may pose a threat  to  our  employees  or  other  pets  in  our  care. This  includes  aggression  problems,  health  problems,  and parasite problems other than fleas.

– Although accidents are very rare, there is a risk when dealing with pets. Hot Shot Dog Grooming will inform you immediately of any incident that occurs or any condition that we notice. Your pet’s safety and comfort is our number one priority.

– ALL PETS MUST BE CURRENT ON VACCINATIONS BEFORE GROOMING.

 

By signing below, you indicate that you understand and agree to our terms of service; that you understand and agree to release and hold Hot Shot Dog Grooming, its owners, employees and affiliates from and against any and all liabilities, expenses, damages, and costs (including attorney fees) resulting from any service provided or injury (including death) to your pet(s) while in our care or afterward. You further authorize Hot Shot Dog Grooming to transport your pet to a veterinarian in the event of an  emergency, and agree that  any  resulting veterinary bills will be your sole responsibility. You grant us permission to seek veterinary attention

MATTED DOG RELEASE FORM

 

Matted coats can cause a variety of skin & health problems.  Matted fur does not allow for air circulation to the skin, causing hot spots, bacterial and fungal infections.  Fleas, ticks, maggots and other parasites may be lurking in the coat causing further skin infections.  Matted fur also pulls and binds, causing pain to your pet when they move or lay on mats.  The skin underneath is usually raw and inflamed.  Matted coats will not dry properly and can lead to rotting fur and skin.

 

Due to matting, your dog’s hair may need to be shaved extremely short to the skin and may show signs of skin irritations and sores due to matting, wet undercoat and dirty coat.  In order to take care of a dog that has not been groomed often enough, and is matted, the required grooming sessions may be long, stressful, or painful(please note, more than 1 grooming session may be required if your dog is severely matted). It is the goal of Hot Shot Dog Grooming and our groomers to make your pet’s grooming experience as stress free as possible.

 

Due to the condition of your dog, Hot Shot Dog Grooming has determined that the only humane thing to do is to shave the dog’s coat down. This is a short clip, removing all the matting.  Where feasible, we work to de-mat/brush out the dog’s coat, but only when it would not cause excessive pain and suffering to attempt to do so. There is a strong chance that your pet’s skin will become irritated during the clipping process.  We will use a medicated shampoo to help soothe any irritation that may occur.  The matted hair rests tightly against the skin, the only way of removing mats is to use a short blade to clip between the skin and mats.  Your dog may be nicked and cut because the groomer has to work so closely to the skin to remove the matted coat.  Our groomers are very careful, but the possibility of injury exists.  Understanding these risks, Hot Shot Dog Grooming is authorized to proceed with the de-matting process. Hot Shot Dog Grooming will be happy to show you how to care for your pet’s coat so that matting does recur.  Please schedule a follow-up appointment to speak with our groomers about the best methods for maintaining your dog’s coat.

 

I hereby release Hot Shot Dog Grooming from any liability associated with the above mentioned process and any and all medical problems that may be uncovered and/or occur during the stripping and de-matting process. Should my pet need veterinarian care after or during the process, I agree to pay any and all veterinarian fees.
